On the left is a schematic of our ORDER FORM in "PDF" format.  There are many ways to submit an order to our office.  You will need ADOBE ACROBAT READER, a free, small software program easily downloaded from www.adobe.com   In fact, many of our clients request PDF versions of completed appraisals as this provides them will full-color documents which print perfectly formatted on any printer, color or black and white.

1.  Click HERE to download this form and SAVE IT to your computer.  PRINT any copy(ies) of this form, complete by hand, and send via FAX to our office.

2.  OPEN the form, and complete each field with your keyboard and mouse.  Save each new order with a unique name and E-MAIL the completed form as an attached document.  (Keyboard editing of PDF forms requires Version 5.0 of Acrobat Reader..
